The mission of this important program is to improve accessibility to healthcare for uninsured and underinsured individuals living in the central and south Berkshire region.
If you do not have health insurance or feel that your insurance is inadequate, come to one of our HCAP free health screenings held across South Berkshire. There, Luci Leonard, LPN, will check your blood pressure, cholesterol and blood glucose. Results are provided immediately! In addition, Octavio Hernandez, bilingual outreach worker, will assist you in enrolling either in MassHealth (Medicaid in Massachusetts) or in Commonwealth Care, a new health insurance program that connects uninsured Massachusetts residents with approved private health insurance, and helps pay for the cost of premiums.
Luci and Octavio, a knowledgeable team of health and outreach workers, will provide you with health education, connect you with a doctor, and help you with transportation for doctor's visits, if necessary. Their typical monthly schedule includes stops in many South Berkshire towns.
CHP Healthy Communities Access Program is funded by the Massachusetts Executive Office of Health and Human Services, the Commonwealth of Massachusetts, University of Massachusetts, Worcester, Elder Services of Berkshire County, Inc. and the Massachusetts Executive Office of Elder Affairs,
and other grants and donations. Additionally, CHP Healthy Communities Access Project is funded in part by a grant from the Blue Cross Blue Shield of Massachusetts Foundation. Created in 2001, the mission of the Blue Cross Blue Shield of Massachusetts Foundation is to expand access to health care in Massachusetts through grantmaking and policy initiatives.
