The CHP Care Coordination Program (CCP) is a free cardiovascular and breast, cervical, and colon cancer screening program for women. CHP implements this health program in collaboration with Berkshire Health Systems. CCP services are available to women who meet specific eligibility criteria:
- Massachusetts residency, and
- 40 - 64 years of age¹, and
- Willing to enroll in the program and become a patient of CHP, and
- Either uninsured or with insurance²
1 Exception: female patients 21-39 years of age, with a diagnosed abnormality such as: breast lump, abnormal pap, a pre-cancerous condition, or a diagnosis of cancer.
2 Insured patients may not qualify for health screenings except for navigation, risk reduction education, and case management.
